Celebrating Innovation & Excellence at Gosport Central PCN

Recognition of hard work and outstanding contributions across the Gosport Central Primary Care Network (PCN).
The Gosport Central Pharmacy Technician Team has been recognised nationally for their innovative approach to cardiovascular prevention. By developing a personalised video and messaging system to educate patients about their QRisk2 score and statin options, the team has transformed patient engagement and reduced consultation times by 95%. This pioneering project is now being shared regionally as a scalable model for preventative care.
As a result of this innovation:
- The team has been shortlisted for Pharmacy Technician Team of the Year by the Association of Pharmacy Technicians UK (APTUK).
- They are also finalists for the Health Service Journal (HSJ) Patient Safety Education and Training Award.
Individual recognition also goes to three members of our Gosport PCN team, all finalists in the prestigious Hampshire and Isle of Wight Healthcare Hero Awards:
- Dr Michael Norris, Lead Clinical Pharmacist – Accountability and Excellence Award Finalist.
Mike’s leadership, creativity, and patient-centred approach have been instrumental in improving access to essential care. His co-development of the QRisk video has enabled thousands of patients to access statin therapy at scale.
- Ashleigh Perryman, Lead Pharmacy Technician – Rising Star Award Finalist.
Since joining in March 2024, Ashleigh has transformed the culture of the pharmacy team and played a key role in innovation and improvement projects, including the QRisk video and a Health Innovation C-Difficile project. Her compassionate leadership and dedication have made a lasting impact on staff and patient outcomes.
- Clare Wood, Community Matron and Frailty Lead – Leading with CARE Award Finalist.
Clare has transformed frailty care within just one year by bringing together a multidisciplinary team and launching the successful 'Frailty Carousel' clinic. Her leadership has empowered staff, improved patient care, and prevented potential hospital admissions for some of our most vulnerable community members.
We are incredibly proud of our teams and individuals whose passion, innovation, and commitment continue to enhance patient care across Gosport. Congratulations to all our finalists and teams – your hard work is truly making a difference.
