Pharmacy Care Coordinator in a Primary Care Network (PCN)
Pharmacy Care Coordinators play a vital role in supporting the smooth and effective delivery of pharmacy services across the PCN. Working closely with Clinical Pharmacists, Pharmacy Technicians, GPs, and the wider healthcare team, they act as a central point of contact for both patients and professionals when it comes to medication-related care and coordination.
Their key responsibilities include:
- Coordinating medication reviews and structured medication appointments for patients, especially those with long-term conditions or complex needs.
- Communicating with patients to ensure they understand their medications and feel supported in managing them effectively.
- Liaising between practices, community pharmacies, and care providers to streamline services and reduce delays or duplication.
- Tracking and monitoring medication-related interventions, ensuring follow-ups are completed and actions are recorded accurately.
- Monitoring medications such as DMARDS and Lithium.
- Supporting safe transfer of care, such as after hospital discharge, by ensuring medication changes are updated and communicated promptly.
